Scott Siewert is a Level II IPTPA Certified Pickleball Teaching Professional who plays at an advanced level. He is offering two clinics at Richardson Community Center with a maximum capacity of 12 players each. Pending final approval from RCC, the dates will be
Beginners-Intermediate clinic – Sept 8 , 6 – 8 p.m.
Intermediate-Advanced clinic – Sept 15, 6 – 8 p.m.
Cost will be $18 collected at the event.

Waiver Agreement *

I have read and agree to the terms and conditions of the waiver.

I recognize and acknowledge that when participating in Columbia County Recreation or Lake City Pickleball events and activities that there are certain risks of physical injury or infection to participants. I agree to assume all risks, and accept personal responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death). This includes any illness, damage, loss, claim, liability or expense, of any kind or nature, that I may suffer arising out of or in connection with becoming exposed to or infected by COVID-19 while participating in an event at the Richardson Community Center. I hereby release, covenant not to sue, and forever discharge the Columbia County Recreation Department and RCC/AMN, its employees, agents, and representative, of and from all liabilities. I waive and relinquish all claims that I or my insurer may have against the officers, agents, servants and employees from any and all claims from injuries, damages or loss which I may have or which may accrue to me on account of my participation in the above program.
